Ryan White Assistance Heading link
RWHAP funds are intended to support only program related needs of eligible individuals.
Eligibility for Ryan White programs is determined under the following guidelines:
- Residency – 15 county service area in the State of Illinois
- Proof of status
- Proof of Insurance
- Proof of Income
- Photo ID
Eligibility is re-certified every 12 months. PHS follows HRSA guidelines by offering a discounted sliding fee schedule for individuals who have no payment source. If you are unable to pay the balance due after the discount is applied, a nominal fee may be accepted.
