PHS exists to provide state of the art, comprehensive care and services with acceptance and respect to individuals and communities in Central Illinois. This mission will be fulfilled through:

At PHS we believe it is possible to live healthy and live well. Our range of health care and support services will help you learn how.

[medical service]

PHS is dedicated to providing the highest quality of care in a patient-centered environment. Our medical team and support staff are committed to treating you with courtesy, compassion, and respect. The focus of our primary care services is to maintain health through routine appointments, wellness education, immunizations, and referrals to other medical specialists if appropriate. Additionally, primary care provides individual interventions to those who are having difficulty with adhering to medications.

[medical case management]

PHS has a team of ten Medical Case Managers to coordinate client-centered services that reduced barriers to significant engagement in care. To do so, they complete a comprehensive assessment and care plan for services and referrals. With this intensive service, it is our hope that medical care and treatments are more easily accessible to clients, that clients are able to successfully engage in their own health care, and that those who face challenges are retained in services long term.

[peer education]

Armed with skills and information, peers are often the best communicators of prevention and harm reduction information. Peers will assist with navigating the healthcare and social service systems. Support provides ways to survive and thrive, which is an important source of inspiration and support for those just beginning to deal with their health status, or searching for tools to improve their quality of life.

[outreach]

PHS offers prevention counseling as well as testing and education programs that address the specific needs of different parts of our diverse communities. We will gladly arrange a presentation to meet your organization or group's individualized needs.

[advocacy]

Client representatives are persons who share client concerns and needs within the counties where they reside. The client representatives act as the liaison between the local community and the Ryan White Part B Advisory Council. At council meetings they speak, act, and/or vote on behalf of the community in their local area. They provide feedback on issues and concerns, while making recommendations to the Part B Advisory Council. PHS currently has two persons volunteering in this federally mandated position.

[mental health services]

Positive mental health is important for all individuals at every stage of life. PHS has a licensed mental health counselor available for individual, couples, and family therapy sessions. Mental health refers to emotional, psychological,and social well-being. It is an important part of staying healthy. Mental health affects how a person thinks, feels, and acts. It also helps determine how one handles stress, relates to others, and makes choices. Positive mental health allows you to:

  • Realize your full potential
  • Cope with the stresses of life
  • Work productively

[ryan white programs]

RWHAP funds are intended to support only program related needs of eligible individuals.

Eligibility for Ryan White programs is determined under the following guidelines:

  • Residency – 15 county service area in the State of Illinois
  • Proof of status
  • Proof of Insurance
  • Proof of Income
  • Photo ID

Eligibility is re-certified every 6 months. PHS follows HRSA guidelines by offering a discounted sliding fee schedule for individuals who have no payment source. If you are unable to pay the balance due after the discount is applied, a nominal fee may be accepted.
